How can I schedule an appointment?

Simply call the office to schedule an appointment. Please have all of your insurance information available , including your insurance card. We can perform a insurance check when you call. Then you can select the date you'd like to visit. It's really that simple. However, don't forget to bring all your insurance card and photo ID with you to the appointment.

How much will it cost?

Our initial office visit starts at $95 US and this does not include x-rays, photos, procedures or any extra supplies/products that the doctor deems necessary at the time of visit (ex. crutches). The $95 covers that initial examination only. The office is available to take cash paying clients,, as well as, those covered by insurance.

Is the doctor available to take my call?

If you leave a message during business hours with staff or on voicemail, the doctor will return your call within 24 hours.

Who should I call about billing questions?

Questions can be answered by our office staff members Lou or Cindy. You may also call the number for the billing office located on your bill.
